Eligibility
To qualify for FMLA leave, you must:
Have been employed by Alto for at least 12 months (not necessarily consecutive)
Have worked at least 1,250 hours during the previous 12 months
Work at a location where 50 or more employees are employed within 75 miles
Entitlement
Eligible employees may take up to 12 weeks of unpaid, job-protected leave in a 12-month period for the following reasons:
The birth, adoption, or foster placement of a child
To care for a spouse, child, or parent with a serious health condition
For your own serious health condition that prevents you from performing essential job functions
For qualifying military family exigencies
Employees caring for a covered service member with a serious injury or illness may take up to 26 weeks of leave in a single 12-month period.
Health Benefits & Job Protection
While on FMLA leave:
Your group health insurance continues under the same terms as if you were working.
You are generally entitled to return to the same or an equivalent position at the end of your leave.
Notice Requirements
When leave is foreseeable, provide 30 days’ notice.
If leave is unforeseeable, notify your manager or HR as soon as possible.
You may be required to provide medical certification verifying the need for leave.
Pay and Accrued Time
FMLA leave is unpaid, but employees must use any available paid time off concurrently. Leave due to workers’ compensation or short-term disability will also run concurrently with FMLA leave.
Important Notes
Taking another job while on any approved leave is grounds for termination, unless approved by Alto.
Employees who fail to return after the approved leave period may be considered to have voluntarily resigned.
