If you do not qualify for FMLA, Alto may approve a personal leave of absence under certain circumstances.
Key Details
Must be requested at least two (2) weeks in advance, or as soon as practicable in emergencies
Typically limited to eight (8) weeks
May be extended with written approval from management and HR
Time away is unpaid, and no vacation or sick time accrues during leave
You are responsible for arranging insurance premium payments with HR during your leave
Reinstatement to your original role is not guaranteed, but Alto will make reasonable efforts to return you to a similar position
Request Process
To request an unpaid personal leave:
Complete the Unpaid Leave of Absence Request Form (located at the bottom of page).
Submit the form to HR here.
If the requested dates change, a new form must be submitted
Notify HR one week prior to your anticipated return date
Important Notes
Taking another job while on any approved leave is grounds for termination, unless approved by Alto.
Employees who fail to return after the approved leave period may be considered to have voluntarily resigned.
